Please refer to the attachments containing the individual entry forms for the forthcoming AWF U15 & Youth Championships that will take place at Sydney Olympic Park, Homebush on 9-10 July, 2016.

Individual entry forms must be completed by those athletes who are eligible to contest the event/s and need to be submitted to the respective State Association office via email together with proof of payment no later than Monday 23rd May, 2016.

Once in receipt of all individual entry forms and payments each State Association is required to submit all entries no later than Monday 30th May, 2016 via its AWF Admin Page adhering to the following steps:

• Log into the AWF System at http://old.awf.com.au/siteadmin/login.aspx
• Click on Australian Championship Nomination
• Choose the applicable Championships
• Then select and add all athletes that will be representing your State
• entries can be saved anytime by clicking Save Registrations
• entries can be finalised by clicking Finalise Payment
